The conclusions were, for years, effectively unexplainable even to the organisations running them. There was no correction mechanism running in the opposite direction, so errors accumulated in only one direction, the profitable one.
Every element that makes a facial recognition story or a content moderation story front-page material is present here, at a scale most of those stories never reach. It got covered as accounting because money was the visible consequence, and the affected people were elderly, dispersed, and unaware.
What the correction actually established
The regulatory response is where the underreporting becomes genuinely costly, because it sets precedents that will matter well beyond healthcare. The framework now consolidating in this corner of healthcare establishes four principles worth naming in public.
